Progression of the Deep Inferior Epigastric Perforator Flap Breast Reconstruction Technique at a Dedicated New Zealand Tertiary Plastic Surgery Unit: a Case Series Analysis

چکیده

Abstract The deep inferior epigastric perforator (DIEP) flap is the gold standard in post-mastectomy breast reconstruction. Improved patient outcomes, reduced operating time and complication rates are reportedly observed over consecutive DIEP series within surgical centres. aim of this study to investigate whether outcomes following reconstruction improve time, by assessing two cohorts undergoing procedure at a microsurgical unit seven years apart. A case analysis was undertaken identify all patients who underwent primary 2011 2018. Outcomes recorded included clinic appointments, number operations, additional operations procedures, along with co-morbidities, BMI radiotherapy rates. Immediate versus delayed unilateral bilateral also recorded. Seven 29 There significant reduction performed 2018 ( p -value = 0.007) procedures 0.043). When adjusted for an outlier, total significantly shorter 0.018), reconstructions. cohort had lower fewer appointments. This illustrates how can experience more complex requires investment terms skill acquisition operative but balanced improvement outcomes.

Breast reconstruction with the deep inferior epigastric perforator flap: history and an update on current technique.

Perforator flaps allow the transfer of the patient's own skin and fat in a reliable manner with minimal donor site morbidity. For breast reconstruction, the abdomen typically is our primary choice as a donor site. The deep inferior epigastric perforator (DIEP) flap remains our first choice as an abdominal perforator flap and has become a mainstay for the repair of mastectomy defects. It allows ...

The Impact of Perforator Number on Deep Inferior Epigastric Perforator Flap Breast Reconstruction

BACKGROUND Perforator flaps minimize abdominal site morbidity during autologous breast reconstruction. The purpose of this study was to assess whether the number of perforators harvested influences the overall deep inferior epigastric perforator (DIEP) flap survival and flap-related complications. METHODS A retrospective review was performed of all DIEP flaps performed at the Hospital of the ...

A Preoperative Marking Template for Deep Inferior Epigastric Artery Perforator Flap Perforators in Breast Reconstruction

Preoperative perforator marking for deep inferior epigastric artery perforator flaps is vital to the success of the procedure in breast reconstruction. Advances in imaging have facilitated accurate identification and preselection of potentially useful perforators. However, the reported imaging accuracy may be lost when preoperatively marking the patient, due to 'mapping errors', as this relies ...

Microcirculatory Evaluation of the Abdominal Skin in Breast Reconstruction with Deep Inferior Epigastric Artery Perforator Flap

UNLABELLED No studies have assessed the perfusion of the undermined abdominal skin in breast reconstruction with deep inferior epigastric artery perforator flap. A greater understanding of the procedure's impact on the perfusion of the abdominal skin can be valuable in predicting areas susceptible to necrosis. METHODS Microcirculatory changes were monitored in the abdominal skin of 20 consecu...

Lymphatic Drainage of the Deep Inferior Epigastric Artery Perforator Flap after Breast Reconstruction

Background: Transplanted tissue in breast reconstruction is routinely performed without any lymphatic anastomosis. Edemas might be observed at the level of these transplanted tissues. New lymphatic routes are supposed to develop but little is known about the development of new lymphatic vessels in the transplanted tissue after free flap transfer. The aim of the study was to evaluate the lymphat...
